HBF Active Connect is an innovative program helping HBF members set and achieve their own, personalised goals to get more active and socially connected. The program is specifically designed to provide tailored support for all ages and activity types.

HBF Active Connect was developed and delivered by BDW, providing a highly-personalised preventative health support service to HBF members. The program included individual telephone support over 2 – 4 coaching sessions, plus unique referrals to local opportunities to get more active and socially connected. For these referrals, BDW developed a database of over 2,500 local activity opportunities across WA, and an easy to use map tool to quickly find the best-fit options for each program participants.
